Tailored Support and Advocacy for Holistic Wellbeing

Main purpose

To adeptly oversee an assigned caseload tailored to the unique needs, strengths, abilities, and preferences of clients, whether through satellite or unmanned services, is our primary focus.

Our comprehensive approach involves conducting thorough assessments, continuous monitoring, strategic planning, facilitating linkages, and providing advocacy to ensure our clients receive holistic support within the service. This includes understanding the intricate service delivery landscape and being well-versed in the resources available for care leavers, aligning with government initiatives to enhance clients’ work readiness.

Our commitment extends beyond routine assistance – we strive to create an environment where clients feel secure and supported, fostering an atmosphere that maximizes their independence, upholds their dignity, and promotes freedom of choice, aligning seamlessly with the overarching goals of the service.

In addition to general support, we specialize in offering tailored advice and assistance, or directing clients with mental health and substance misuse needs to relevant resources. This ensures that each individual receives the specialized care and guidance necessary for their unique circumstances, further enhancing the effectiveness and impact of our service.
